How to Register as a Patientregister_now

If you are new to the area and wish to register with this practice, please ask at reception.

If you have your medical card then please bring this along with you. We will also require some other means of identification, photographic and a form of I.D with your current new home address on it such as a council tax bill.

You will be issued with a welcome pack from our Reception team and included in that will be an option for you to book a well man/woman check with our practice nurse should you wish to do so.


Practice Area

Our practice area denotes the boundary for the Home Visiting Service for those patients incapacitated by illness or disability. Patients outside our practice area may still register at The Surgery for convenience, such as commuters who find it easier to attend a Health Centre throughout the working day and journey. Patients wishing to register from outside the boundary are not eligible for our Home Visiting Services, those will be undertaken by the 111 Service if ever necessary.
